If your child requires medication while they are with us, please ask for an Administration of Medication Consent Form from the Playleader.
We are unable to administer medication without this form. We are unable to administer the first dose of any medication and parents/carers should have already given one dose to ensure the child does not have an adverse effect.
A new form is required for each change of medication/treatment.
A copy of the Administration of Medication policy is available on request.
